While tramadol has lower potency than traditional opioids, its balanced efficacy and safety make it a practical option for pain management in various patient populations.

All respondents considered tramadol to be an important or highly important medication in the treatment of moderate to severe non-cancer pain in the United States. It is viewed as an alternative to NSAIDs or strong opioids, particularly for elderly patients or individuals with hepatic or renal impairment. Its efficacy in mixed pain (nociceptive and neuropathic components) and favorable benefit–risk ratio during prolonged use were consistently highlighted.

Furthermore, respondents noted that NSAID-related safety concerns often limit their long-term use, especially in chronic pain and geriatric patients. As a result, tramadol is frequently prescribed as a safer alternative, a trend consistent with current literature (Figure 2).

The primary reasons for prescribing tramadol, as reported by healthcare professionals, include its efficacy, safety, good tolerability, and low incidence of adverse drug reactions compared to other opioids. The widespread use of tramadol reflects its multimodal mechanism of action—combining opioid receptor activity with monoaminergic modulation—which contributes to its broad effectiveness.

The findings confirm that tramadol is effective in both acute and chronic pain, including nociceptive and neuropathic types. This makes it particularly useful for managing complex pain syndromes such as cancer-related pain and chronic low back pain, which often involve mixed pain mechanisms. In this study, a total of 740 patients were included. Based on patient interviews and medical histories, 43% reported using tramadol more than three times within a year, 38% used it twice or more, and the remaining participants were first-time users who reported positive outcomes and effective pain relief (Table 1).

Discussion

Tramadol has become one of the most frequently used analgesics in the management of moderate to severe lower back pain. It is readily available, cost-effective, and has a favorable safety profile. The survey findings indicate that its continued utilization among healthcare providers is influenced by several key factors:

  • Proven efficacy in both cancer-related and non-cancer pain, confirmed by healthcare professionals participating in this study.

  • Superior safety and tolerability compared to NSAIDs and stronger opioids.

  • Ease of storage and prescription compared to controlled opioid medications due to its lower risk classification.

  • Suitability for use as a step-down analgesic following major surgery and patient-controlled or epidural analgesia.

An experienced orthopedic surgeon with over 25 years of clinical practice emphasized that “tramadol is also effective for chronic pain conditions such as sciatica and rheumatoid disorders when standard analgesics fail to provide relief. It is often used in combination with other analgesics and muscle relaxants to enhance therapeutic response.”

The expert responses clearly demonstrate that tramadol is a valuable and flexible option for managing moderate to severe pain of various origins, especially musculoskeletal and postoperative pain. It is typically prescribed when non-opioid agents such as acetaminophen, NSAIDs, or COX-2 inhibitors provide inadequate analgesia, are poorly tolerated, or are contraindicated, and when the use of stronger opioids is not justified or advisable.

This therapeutic role is consistent with national and international guidelines, as well as the World Health Organization’s recommendations for pain management. Tramadol is frequently selected as an alternative to NSAIDs because of its lower incidence of renal and gastrointestinal side effects, which explains its common use among older adults and patients with comorbidities.

The study population included more women than men, particularly among those who used tramadol two or more times per year. Older adults represented a substantial portion of the sample, reflecting the higher prevalence of low back pain among women and individuals aged 40 to 99 years.

The increase in tramadol prescriptions in surgical and orthopedic departments suggests that it is often prescribed to achieve adequate postoperative pain control, particularly in patients with higher pain intensity. This trend supports the association between surgical procedures and the need for tramadol online use in emergency and postoperative care settings.

Conclusion

Tramadol and tramadol-containing analgesic combinations play an important role in the pharmacologic management of moderate to severe pain. For many patients, tramadol represents a stronger yet safer alternative to high doses of NSAIDs or lower doses of potent opioids such as morphine or oxycodone. Healthcare providers frequently prefer tramadol due to its balanced efficacy, safety, and lower risk of dependence compared to other modern opioids.

Overall, the findings support the continued use of tramadol as a recommended option for managing moderate to severe lower back pain and other chronic pain conditions in clinical practice across the United States.
